Personal Care Worker

REPORTS TO:  HH Operations Manager

STATUS:  Full/Part-Time; Hours/Week Varies; Non-Exempt/Hourly

Position Summary: Provides personal care services under the direction of IHSS Staff and within the guidelines of The IC’s Home Health department.

Minimum Qualifications:

  • Be at least 18 years of age
  • Current driver’s license, valid vehicle insurance, safe driving record and reliable transportation
  • Current CPR certification
  • Current TB screening (provided upon hire)
  • Successful completion of the PCW general training program, skills test, competency evaluation, or the equivalent documentation (provided upon hire)
  • Demonstrates THE IC’s core values of Inclusiveness, Flexibility, Person First, Accountability and Integrity and essential team virtues of Humble, Hungry and Smart

Essential Functions/Areas of Accountability:

  • Performs simple procedures as are described in the PCP scope of practice as an extension of HCBS under the direction and supervision of the HCBS Coordinator, to include personal care activities including but not limited to:
    • Bathing: Bed bath/tub bath/shower
    • Shampooing
    • Skin care/nail care/shaving with electric razor
    • Assist with toileting
    • Reminders for self-administered medications
    • Assist with walking, simple positioning
    • Oral hygiene
    • Dressing
    • Help with exercises if client can move unassisted
    • Protective oversight
  • Performs household services essential to health care at home, as indicated on the care plan including but not limited to:
    • Meal preparation/feeding
    • Protective oversight
    • Laundry duties
    • Light housekeeping
    • Grocery Shopping
    • Cleaning durable medical equipment
  • Reports observed or reported changes in the client’s condition and/or needs to the supervisor
  • Completes the appropriate records to document care given and including all pertinent observations
  • Checks EMR for next weeks’ schedule every Friday by 10 AM.  Notify scheduler immediately of errors in the schedule
  • Utilizes EMR to chart arrival and departure time as well as entering task codes to document care provided. Notifies office staff of difficulties encountered with EMR while in the clients’ home, or the next business day if event occurs after hours. Records accurate time in/out, using EMR, for every visit
  • Charting must have the travel time documented on each visit as pertains to client
  • Promotes personal safety and a safe environment for consumers by observing infection control practices. Follows The IC guidelines and report unsafe situations to the supervisor
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings as required and team conferences as requested
  • Is required to complete ongoing education or training to remain proficient in client’s care. This will include a yearly competency evaluation
  • All care providers will not provide transportation services and will not provide financial management services
  • Complete 6 annual Continuing Education Units (CEU)
  • Maintains confidentiality as outlined in The IC Employee Handbook and work within the confines of HIPAA regulations
  • Participates in The IC’s ongoing quality improvement activities
  • Maintains CPR certification

Physical/Environmental Requirements:

  • Home health field environment; must be able to lift, push, pull up to 50 lbs. frequently
  • Ability to travel within Colorado Springs and surrounding counties to meet with individuals, including accessing consumer homes, attend meetings and present information
  • To drive any vehicle in the course and scope of employment, an employee must maintain an acceptable Motor Vehicle Record (MVR) as determined by The IC’s liability insurance carrier. IC employees will be subject to reasonable suspicion, post-incident, and bi-annual random MVR checks to determine driving acceptability
