Staff Coordinator

REPORTS TO: HH Operations Manager

STATUS: Full-time; 40 hour/week; non-exempt/hourly – rotational on-call responsibility

Position summary:

The Home Health Staff Coordinator is responsible for providing support services and assistance to the Home Health Operations Manager with the primary role of serving as a mentor and resource to The Independence Center (The IC) caregiver staff to improve caregiver experience.  Works closely with the Orientation and Training Specialist and Human Resources.

Minimum Qualifications:

  • High school diploma or GED.
  • Current Colorado CNA license required.
  • Current CPR certification.
  • Two years of experience in the home healthcare industry.
  • Desire to work with people with disabilities.
  • Current driver’s license, valid vehicle insurance, safe driving record, and reliable transportation.
  • Proficient with computers, including Word, Excel, PowerPoint, Publisher, Outlook, Teams, and Zoom.

Preferred Qualifications:

  • Some college education is desirable.
  • Staff scheduling experience preferred.

Essential Functions/Areas of Accountability:

  • Assists with Visits Scheduled Not Made (VSNM), including pulling daily reports, following up with caregivers, and documenting caregiver communications.
  • Provides support to HH Schedulers with maintaining schedules and assisting with field staff to ensure coverage of client needs.
  • Assist HH Schedulers with the payroll process, including managing administrative activity, performing daily data validation, running actual vs. scheduled visits, and making corrections/requests with time verification from field staff.
  • Provide administrative support to HH managers for IC forms, provide updates, and document caregiver performance and compliance.
  • Provide Home Healthcare Field support by performing CNA, PCA, PCW, and HMK visits which includes on-call visits.
  • Manages the on-call phone in accordance with the CNA Scheduler on-call policy
  • Assist HH Managers by screening incoming applications and inquiries and scheduling interviews.
  • Assist HH Orientation and Training Specialist with orientation and serving as a backup to the HH Orientation and Training Specialist.
  • Assist with caregiver onboarding by providing training to new hires on the Electronic Medical Record (EMR) process, setting up new caregivers in EMR, and being the primary point of contact when they meet new consumers and encounter issues.
  • Provide caregiver support by offering extraordinary customer service, troubleshooting EMR or website issues, assisting with in-service completion, and maintaining an open dialogue with caregivers.
  • Assist with caregiver compliance with division standards such as proper documenting and time recording, ensuring quality care to consumers, and following up with caregivers on renewable items
  • Comply with applicable state and federal standards, The IC policies and procedures, and community and professional standards.
  • Accept responsibility for personal and professional development.
  • Maintain Colorado CNA License & CPR certification.
  • Complete 12 annual Continuing Education Units (CEU).
  • Attend all required Home Health meetings.
  • Participate in The IC’s ongoing quality improvement activities.

Physical/Environmental Requirements:

  • Frequently lift, push, pull, up to 25 lbs., and occasionally lift, push, pull,  up to 50 lbs.
  • Home health field environment with a high level of contact with others, often requiring personal protective equipment. A considerable amount of time is spent walking and bending.
  • Office environment requiring ongoing computer use and extended amount of time sitting or standing.
  • Internet access to work remotely if protocols are issued for remote work.
  • Must maintain an acceptable Motor Vehicle Record (MVR) as determined by The IC’s liability insurance carrier.  Employees who are required to drive for the agency in the course and scope of employment will be subject to reasonable suspicion, post-incident, and bi-annual random MVR checks to determine driving acceptability and must maintain an acceptable Motor Vehicle Record (MVR) as determined by The IC’s liability insurance carrier to maintain employment.

Pay Range: $16-$18 per hour
