Our Outreach Specialists are available to meet with you in person, either at a designated location or at your home. With a unique understanding of resources in your area, they can connect you or a loved one with services and information to help you live independently in your own home and community.

Where does our Outreach Team serve?
The Outreach Team serves consumers in SIX counties here in Colorado: El Paso, Teller, Park, Lincoln, Kit Carson, and Cheyenne.
Who does the Outreach Team serve?
We work with people of all ages and disabilities. Consumers younger than 18 years old will need a parent/guardian permission form to work with us.
How does the Outreach Team get resources to its consumers?
We bring The Independence Center’s resources to you! Meeting you where you are helps the team understand local barriers and resources needed to achieve your goals. We do not transport consumers.
The Outreach Team also works with local partners who provide spaces where we can meet with you.
Can The IC’s outreach team provide me a case manager?
We do not offer case management. However, we can help you locate a case manager if needed. We work closely with team members at The IC main office and local community organizations to help consumers gain the information and skills needed to complete their goal.
